Scheduling Appointments

To make an appointment with one of our physicians, please call (970) 249-6641, Monday through Friday between 8:00 AM and 5:00 PM. If you cannot keep a scheduled appointment, please notify our office as soon as possible.

New Patients

1. Patient Health History Forms (Patient Portal)

Please click here to open the Patient Health History forms. Once you have completed this form, the results will be forwarded to our office and added to your electronic medical record. Please answer all questions thoroughly and accurately. You will have 60 minutes to complete the form before the session times out.

Patients who are new to WSO or have not visited our offices in more than a year should arrive 15 minutes before their scheduled appointment time to complete their registration.

2. Patient Insurance Forms and Policies

For your convenience, the patient insurance forms and policies can be printed. Please complete and bring it to your appointment. Please bring the following:

  • Insurance card
  • List of current medications and medical conditions
  • Primary-care physician address and phone number and contact information for other physicians involved with your care
  • Recent diagnostic images including CTs, MRIs, x-rays, bone scans, or EMGs. Bring both original films and copies of reports.

Participating Insurance Plans

Western Slope Orthopaedics files all claims to insurance companies within the continental United States as a service to you. To assure accurate processing of services, we ask for your assistance in providing accurate insurance information to us. Therefore, please bring your insurance card and any required referrals with you to each visit. Please be prepared to take care of your co-payment at the time of your visit. If you have a work injury, please call the office with the date of injury, your employer’s name, and workers’ compensation insurance information before booking the appointment so that we may obtain approval for treatment and payment of our fees. If you have a motor vehicle injury, please bring all relevant information to your first visit.

Your medical insurance represents a contract between you and your insurance carrier. Depending on your individual benefits, your insurance carrier may or may not cover the services provided. Ultimately, you are responsible for the final payment or settlement of any outstanding balances. Patients are expected to pay past due amounts at the time of service.
